The health care delivery system in Canada is going through an accelerated period of intense focus and transformation. An integrated Continuum of Care model of healthcare delivery requires that patient information be made available to all health care providers, when and where they need it (taking into account obvious privacy and information security concerns). The building blocks for the Continuum of Care already exist; what’s lacking is the integration and seamless exchange of information between and across these components.
